You can directly access a physical therapist without a physician diagnosis or referral! What does this mean to you? It means you can save time and money and get immediate treatment by seeing a physical therapist first. Call our office to ensure your insurance covers this beneficial service!
FEES
The fees for our services are listed below when you are paying cash at the time of service. At Sebastopol Physical Therapy we will bill for your Physical Therapy services if we are contracted with your insurance company. We are providers for Medicare and Blue Shield. We accept Workers' Compensation Claims. Please call our office to see if your insurance coverage includes our services. When paying cash at the time of visit, we offer the following fee schedule:
- Physical Therapy Evaluation: $175.00
- Physical Therapy Visit: $145.00
- 6 Visit Prepaid Pack: $840.00
Cancellation Policy
We require a full 24-hour notification for any appointment you have scheduled and need to cancel. Fines are as follows: $75 first - $125 second - $165 third. If you no show, you will owe $165 for eval and $155 for regular treatment.
